VBS Registration! — Union Baptist Church

Join us for an amazing adventure highlighting the courageous journeys of several Bible characters while we celebrate the awesome and never-ending love of Jesus!

Kids will enjoy four evenings of Bible lessons, music, snacks, games, crafts, and FUN! The fifth and final night will be Family FUN night, complete with games, a bounce house, food, and slushies! We meet Sunday - Thursday from 6:15 - 8:15 PM.

Medical Release, Liability Waiver, Photo Release, and Consent

Medical Release: I (We), the parent(s) or guardian(s) of the above listed child grant permission for our child to participate in Vacation Bible School at Union Baptist Church of Troy, OH, and to receive emergency medical treatment if necessary. If I (we) or the listed emergency contact cannot be reached, I (we) give our permission to the staff to secure the services of a licensed physician to provide the necessary care for my (our) child’s well-being.

Liability Waiver: I (We) also release and agree to hold harmless Union Baptist Church and all its participants from any liability and assume all risk of injury, damage, or expenses as the result of participation in activities in VBS.

Photo Release: I (We) understand that the uploaded image is to aid the staff in identifying my (our) child and these images will not be posted on the website or Facebook. Also, as a participant in Union Baptist Church’s Vacation Bible School, my (our) child may be photographed or videoed during group events or activities. By checking the box in the photo upload section, I consent to give Union Baptist Church the right to post any group participation images or videos to the website or Facebook that may include my (our) child.
